If you are meeting with one of our enrollment counselors to apply for insurance, please bring the following to your appointment:

  • I.D. for adults
  • Birth certificate, passport, permanent resident card, VISA, or certificate of citizenship
  • Social Security number
  • Most recent monthly income, such as 4 paystubs, letter of employment (if you do not receive paystubs), Social Security award letter, proof of child support, proof of unemployment, or income taxes

Payments

We accept Visa, MasterCard, American Express, cash and checks. We also accept most insurances. If you have insurance, please bring your insurance cards with you. If you don’t have insurance, we can still help you.

Sliding Fee Service

Depending on your needs and eligibility, we can help you access low cost programs. Based on your family income and number of family members in your household, you may also be eligible for sliding fee discount. The following is list of documents which may be accepted for income verification and confirmation of household size.

  • IRS 1040 federal tax return (current year)
  • Medicare/Medicaid or Social Security letter & check stubs
  • Self-employed patients must provide a 1040 with corresponding forms and/or documentation of a profit and loss statement
  • LCDP reserves the right to accept additional forms of income verification

If you have a change in the size of your family or how much money your family makes, please notify the front desk, as this may change the program for which you are eligible.

For those insurance that are not in-network for La Clinica, the patient may be reponsible for the full charge of their visit.

La Clinica del Pueblo prides ourselves in offering Patient Advocacy Services to our patients. These services play a critical role in helping to break down the barriers to healthcare faced by low-income patients, uninsured and underinsured community members and other individuals in need that come to La Clinica.

Promotoras serve as the link to health and social services for many community members in northern Rio Arriba county. They work with individual patients and assist them in learning the knowledge and skills necessary to achieve optimal health and well being.

La Clinica offers the car Seats and has Certified Car Seat Technicians through The New Mexico Child Safety Seat Distribution Program (NMCSSDP).
